What exactly did God mean by this? Does this mean accountability? If so, when does this accountability reach God? It was once explained to be that in a denomination you are accountable to the leaders, then to the deacons, all the way to the CEO/President/Whatever whomever. But is was just a circle of accountability among men. When does it reach God? Does it ever? Should it?

Absolutely. If Jesus Christ is our Lord, we are accountable to be obedient to His commands. And certainly we are to love God with all our heart, soul, mind and strength, and our neighbor as ourselves. Jesus made that clear. James also has made it clear that faith unaccompanied by a life of love and charity is dead. Even Paul said that we are to work out our salavation with fear and trembling, since it is God who is at work in us both to will and to do of His good pleasure. The one who would lead must be a servant of all, and we are called to take up our cross and follow Jesus. I don't believe this simply means to "reckon ourselves dead to sin but alive to Christ". I believe that our lives are to be sacrificial. Far too often, our "Christianity" is merely a utilitarian approach to God--God will give us peace, God will make us happy, God will give us wealth, etc.--rather than the approach of a servant to His loving Lord and Master. Although we are certainly to render respect, and a reasonable obedience, to our Christian authorities, at the judgement seat of Christ we will be held responsible, not for our obedience to church authorities, but our obedience to Jesus Christ.
